Your health
care insurance coverage may cover some, but not
all, of the cost of your treatment. Most plans
have co-payments, deductibles, and other expenses
which must be paid by the patient. Please bring
your insurance card and co-pay with you to your
visit. Please bring a current referral with you
to your appointment, if one is required by your
insurance company.
Even though we do participate with some insurance
programs, it is important to remember that your
insurance is a contract between you and your insurance
company. As such, we do not file claims to commercial
insurance carriers, except as a courtesy to our
hospital patients. We will make every reasonable
effort to assist you. If you have further questions,

We have contracted to participate with the following
companies in their HMO and/or PPO products. Each
physician's participation with individual carriers
is subject to change without notice. Patients
should call their insurance carriers directly
to verify their physician's current participation
status with their insurance plan. Our participation
with these carriers is strictly as pulmonary and
critical care specialists.

If we do not
participate with your insurance plan, payment
in full is expected at the time services are rendered.
VISA, Mastercard, Discover, personal checks, and
cash are accepted. An itemized statement is available
to you as you leave the office for you to file
with your insurance carrier for reimbursement.
If at any time payment poses a financial hardship,
individual arrangements can be made and should
be discussed with our billing staff prior to your
appointment.
Payment for the deductibles, coinsurance, and
co-pays for participating insurance plans is expected
at the time of service. If you have an HMO that
requires a referral, the referral must be brought
with you at the time of the appointment or we
will be unable to see you without payment in full.
