We see all patients on an appointment basis and ask that you call in advance so that we may reserve time for you.
If you are unable to keep an appointment, 24-hour notice is appreciated so that this time may be given to another patient.
 
 
 
If you are visiting one of our offices for the first time either as a new patient consultation or as a follow-up to hospital visits, please bring the following with you:
 
Your x-rays/CT Films
Your current insurance card
A list of your current medications, including dosages, with any refills you need, which the medical assistant will then attach directly to your chart
A referral, valid for one visit, if your insurance requires it
Your driver's license or a current photo i.d.
A copy of the results of any recent breathing tests
Completed patient forms
 
Due to the nature of our practice, please be considerate and do not wear perfume, cologne, or other fragrances to our office, as they may worsen other patients' conditions. Also, please treat our staff kindly and courteously as we are here to help you.

Preparation for methacholine challenge test
 
No short-acting inhaled bronchodilators (Albuterol, Proventil, Ventolin, Xopenex, Pro-Air, etc.) for 8 hours prior to testing.
No medium-acting inhaled bronchodilators (Atrovent) for 24 hours prior to testing.
 
No long-acting inhaled bronchodilators (Advair, Serevent, Foradil, Spiriva, Brovans, etc.) for 48 hours prior to testing.
 
No inhaled steroids (Aerobid, Azmacort, Advair, Asmanex, Flovent, Pulmicort, Symbicort, etc.) for 48 hours prior to testing.
 
No antihistamines (Allegra, Atarax, Cetirizine, Clarinex, Claratin, Fexafenadine, Loratadine, Zyrtec, etc.) for 3 days prior to testing.
No Theophylline, Uniphyl, or Theo-24 for 48 hours prior to testing.
No Cromolyn Sodium (Intal), or Nedocromil (Tilade) for 8 hours prior to testing.
No Leukotriene modifiers for 24 hours prior to testing.
No caffeine the day of the test (coffee, tea, sodas, chocolate).
NO SMOKING 24 hours prior to testing.
DO NOT stop taking prescribed nasal sprays prior to the test.
 
If you do not see your inhaler listed here, please ask if you need to stop using it prior to taking this test.

No Atrovent for 4 hours prior to testing.
 
No long-acting inhaled bronchodilators (Advair, Serevent, Foradil, Spiriva, Brovans, etc.) for 12 hours prior to testing.
No Theophylline, Uniphyl, or Theo-24 for 12 hours prior to testing.
No caffeine the day of the test (coffee, tea, sodas, chocolate).
NO SMOKING 24 hours prior to testing.
 
If you do not see your inhaler listed here, please ask if you need to stop using it prior to taking this test.
 
 
 
Your health care insurance coverage may cover some, but not all, of the cost of your treatment. Most plans have co-payments, deductibles, and other expenses which must be paid by the patient. Please bring your insurance card and co-pay with you to your visit. Please bring a current referral with you to your appointment, if one is required by your insurance company.

Even though we do participate with some insurance programs, it is important to remember that your insurance is a contract between you and your insurance company. As such, we do not file claims to commercial insurance carriers, except as a courtesy to our hospital patients. We will make every reasonable effort to assist you. If you have further questions, please refer to our payment policy or call our billing office at 407-425-6590.

We have contracted to participate with the following companies in their HMO and/or PPO products. Each physician's participation with individual carriers is subject to change without notice. Patients should call their insurance carriers directly to verify their physician's current participation status with their insurance plan. Our participation with these carriers is strictly as pulmonary and critical care specialists.

If we do not participate with your insurance plan, payment in full is expected at the time services are rendered. VISA, Mastercard, Discover, personal checks, and cash are accepted. An itemized statement is available to you as you leave the office for you to file with your insurance carrier for reimbursement. If at any time payment poses a financial hardship, individual arrangements can be made and should be discussed with our billing staff prior to your appointment.

Payment for the deductibles, coinsurance, and co-pays for participating insurance plans is expected at the time of service. If you have an HMO that requires a referral, the referral must be brought with you at the time of the appointment or we will be unable to see you without payment in full.

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully and complete the acknowledgement form.
